Zachary, we agree. Pharmacists play an important role in the patient-centered medical home model and help improve the quality of patients’ care, while saving them money. That’s why with Carolina Advanced Health we created a model that brings together all of a patient’s care under one roof, including a pharmacist to consult with patients about their prescriptions. We hope that this practice can serve as a model for more coordinated care across the state.
